How do I add or remove dependents from my policy?

Visit our Member Resources site to download the Policy Change Form specific to your product. Please follow the submission instructions on the form. Note that there are rules and exclusions for adding and removing dependents. If you have any questions, please e-mail us at enrollment@wellaway.com.

Submit the completed form to enrollment@wellaway.com.
